Epilepsy

Person in a hallway holding their forehead and the wall trying to balance themselves.

Epilepsy is a chronic brain disorder in which groups of nerve cells, or neurons, in the brain sometimes send the wrong signals and cause seizures. Neurons normally generate electrical and chemical signals that act on other neurons, organs, and muscles to produce human thoughts, feelings, and actions.

The connection between epilepsy and body temperature regulation is well established in clinical literature. A review chapter in Elsevier's Handbook of Clinical Neurology (2018) describes a bidirectional relationship: elevated body temperature can reduce the threshold at which seizures occur, while seizures themselves can generate hyperthermia through excessive muscle activity and autonomic nervous system activation. The same review notes that hypothermia may actually prevent or abort seizures in some circumstances, further establishing core temperature as a meaningful variable in seizure management.

A separate body of research has documented that certain anti-seizure medications, particularly topiramate and zonisamide, can impair sweat production, which is the body's primary cooling mechanism, placing individuals at elevated risk of overheating in warm environments or during physical activity. For people on these medications, external cooling support is not simply a comfort measure; it addresses a direct physiological vulnerability.
